Waiver services (Consumer Directed)

• Preparing meals, shopping, housekeeping, laundry
• Providing support at work or in the community
• Money management
• Medical appointment
• Work, School or social activities

• Bathing or other personal Hygiene
• Help with walking
• Help to safely transfer from one position to another
• Help with toileting
• Dressing and grooming

This service is available to individuals who cannot safely be alone and no one in their support system is home.

Available if:

• Individual has medical or physical condition that prohibits them being left alone
• Individual is unable to call for help in an emergency
• No competent adult in the home that can call 911 during an emergency
• Requires this service to ensure their health, safety and welfare

The services facilitator determines if consumer directed services are appropriate for waiver members. 

Key Points

An individual will not be considered for waiver services if health, safety or welfare concerns exist.

The services facilitator cannot provide the individuals care needs

The individual is unable to provide a back-up plan.

An individual plan of care cannot be developed appropriately.
